FRENULUM BREVE

For some men, the frenulum may be too tight. In many cases, it may cause a downward deflection of the head of the penis which can make penetration difficult and painful for both partners. It can also cause bleeding when the penis is erect. The degree of tightness and thickness of the frenulum is very variable.
Frenulum reduction can be accomplished with a frenuloplasty. Using a Y shaped incision, also known as a Y Plasty, the tight band can be relieved without a significant reduction in pleasurable sensation.
For men who have gone through their life suffering from pain and/or bleeding, a frenuloplasty can improve sexual performance without losing normal sensation. It is typically a 30 minute surgical procedure that can be performed under general or local anesthesia.

Surgery Expectations

After the short surgery, there will be some discomfort, but very little severe pain. Anesthetics will reduce discomfort, but may leave the patient drowsy, so driving should be avoided for a period of 24 hours after the surgery. Most patients return home within two hours of the procedure and while there is several sutures involved in the procedure, they usually dissolve or drop out within a week of the procedure. The wound may be washed with soap and water as early as 24 hours after the surgery. Soreness may linger for one or two weeks, but after two weeks a normal level of activity should be expected, with sexual activity possible within two to three weeks. Complications are rare, and are usually no more serious than a slight bleeding in the incision.

Frenuloplasty in Uncircumcised Penis Overview

If an uncircumcised man has a problem with his foreskin (the loose skin that covers the end of the penis) such that it is very tightly attached to the undersurface of the glans of the penis, it can cause difficulty in movement of the skin as well as pain during erections. A foreskin that is tight enough to restrict movement is commonly known as a frenulum breve. This is remedied by a procedure called a frenuloplasty, where the tightness and restriction is removed. While circumcision can help this condition, frenuloplasty is a specialized technique that seeks to treat solely this condition while preserving the foreskin. After a few weeks and the incision healed, the patient will experience reduced or no pain with erections, will have more enjoyable sexual intercourse, and will feel an increase in the range of motion for the penile skin.


Frenulum breve , or short frenulum, is a condition in which the frenulum of the penis, which is an elastic band of tissue under the glans penis that connects to the foreskin and helps contract it over the glans, is too short and thus restricts the movement of the foreskin.


What causes short frenulum?

INTRODUCTION: The presence of a short frenulum is a common cause for dyspareunia in males, often resulting in painful intercourse and trauma to the frenulum.


Signs and Symptoms of Frenulum Breve
A frenulum breve leads to ventral deviation of the glans and pain during erection. Sometimes, tearing of the frenulum causes heavy bleeding. The tearing of frenulum breve occurs particularly during sexual activity of young man.

Diagnostic Work-Up

Frenulum breve is a clinical diagnosis. Important is the exclusion of a phimosis before surgical treatment.

Treatment of Frenulum Breve

Mild compression of the blood supply is usually sufficient to stop the bleeding after tearing of the frenulum. Further treatment depends on the glans mobility after healing. In case of heavy bleeding, hemostatic sutures (after anesthesia) are necessary, frenuloplasty can be done in the same setting.

The Week Before Your Procedure

Starting seven days before the procedure, you must entirely avoid taking any aspirin, NSAIDs, fish oil, or other anti-inflammatory medication. These thin the blood somewhat and increase the risk of bleeding. You can still take other pain relief medication like Tylenol.

You must entirely avoid aspirin and non-steroidal anti-inflammatory drugs such as Ibuprofen, Motrin, Aleve, Advil, Naproxen, Diclofenac, and others because they may thin the blood and cause unnecessary bleeding.

Tylenol (acetaminophen) is acceptable because it does not impact your risk of bleeding during and after surgery. Don’t take aspirin and the other anti-inflammatories for your own safety.

The doctor may not perform your procedure if you have taken these drugs recently. If you have a medical requirement to do so, please discuss with Dr. prior to your appointment.

Preparing for Your Circumcision

Take it easy the night before your procedure. Do your best to have a good sleep the night before and follow your normal breakfast routine (except for minimizing coffee) so that you will feel your normal self. Some patients find it stressful to skip coffee, so if you are one of those people who really needs his coffee in the morning  – maybe just have one cup.

The goal is to minimize your stress, so have a cup of coffee if skipping it is will cause distress. If you have taken the prescribed Valium, you must have someone come with you to drive you home as well. Valium can slow your reaction time, thus it would be dangerous to drive yourself.

The morning of your circumcision it is best if you are both physically and mentally calm. You may bring your headphones and listen to your favorite relaxing music on your phone or other small music player during the surgery, if you wish.
